Enrollment - Health Information and KDHE Guidelines

These documents list health information needed for enrollment and KDHE guidelines that will be used when a child is sick.

Exclusion - KDHE Guidelines

• Exclusion Policy for Sick Students (According to KDHE) - Identifies specific infectious and contagious diseases and return to school guidelines.

• Isolation and Quarantine - KDHE requirements for isolation and quarantine of specific infectious and contagious diseases.

Health Screening Forms

Forms utilized to document the outcome of health screenings.

• Health Assessment Form - Should be completed for every child entering a Kansas school for the first time, up to the age of nine years.
• Hearing Test Referral - Parents/legal guardians will receive a hearing test referral letter when a follow up evaluation is indicated.

• Screening Notice - Results of student health screening including vision, hearing, height, weight, and body mass index.

• Student Health History - School personnel will hold this information in confidence. It is important that the questionnaire be answered completely and accurately each school year.

• Vision Test Referral - Parents/legal guardians will receive a vision test referral letter when a visual problem is suspected.

Illness

See the section on "Exclusion" above for information about conditions for exclusion from school or returning to school.

• Fever - Guidelines when a fever is suspected.

• When Your Child Becomes Sick - Care is taken to see that your child does not remain in school when he/she becomes sick, has a rash, sore throat or temperature over 99.6.
• Cuando su nino se enfeerma (When Your Child Becomes Sick) - El cuidado es tomado para ver que su nino no se permanesca en la escuela cuando el/ella se enferma, tenga un sarpullido, dolor de la garganta o temperatura adoloridas mas de 99.6.

Immunizations (Vaccinations)

• Exclusion From School Letter - Send this letter to parents when the student has incomplete health records and the 90 day grace period has expired.
• Health Assessment and/or Immunization Request - This letter will be sent to parents/legal guardians when the student's immunizations are not up-to-date and/or a health assessment is needed.
• KDHE Immunization Requirements - This communication is to clarify immunization requirements and recommendations for the current school year.

Medications

• Medication Guidelines - If your child is currently receiving medication at school, or should the need arise, please follow these guidelines.
• Pautas de medicina (Medication Guidelines) - Si el estudiante actualmente recibe medicina en la escuela, o si la necesita recibir medicina en el futuro, por favor sigue estas instrucciones.
• Medication Release Form (Epi-pen) - The parent or legal guardian is responsible to submit a completed form when a student will carry an Epi-pen in the school setting.
• Medication Release Form (Glucagon Injection) - The parents or legal guardian is responsible to submit a completed form when a student will carry a glucagon injection in the school setting.
• Medication Release Form (Inhaler) - The parent or lawful custodian is responsible for submitting a completed form when a student will carry an inhaler in the school settings.
• Request to Administer Medication - When a prescription medication is to be given to a student during the school day, the parent or legal guardian is responsible for submitting the completed form.
• Request and Permission to Discontinue the Supervision of Medication at School - Parents/legal guardians should complete this form to discontinue the administration of a medication in the school setting.
• Request and Permission To Supervise the Taking of Medication at School - When an over-the-counter medication is to be given to a student during the school day, the parent or legal guardian is responsible for completing this form.

Special Health Care Needs

• Anticipated Health Crisis Plan - Details the procedures to be followed and the personnel to be involved if or when an emergency occurs.
